Please complete this form before sending samples to our laboratory for analysis. An confirmation email will then be sent to the supplied email address. If required, a chain of custody form will be attached to this email. Please include a hard copy of this chain of custody form with the shipment. Our lab team receives a high volume of samples, and processing may be delayed if samples are received without first being submitted.
